Who Could Be Part of Your Youth Team?

Whether you’re thinking about work, study, friendships, independence or what comes next, you don’t need to figure it out on your own.

If you’re aged 9–25, you can choose for a youth lead practitioner to be your main therapy person. They take the time to get to know you, understand what’s important to you and help bring together the right support around your goals.

Rather than managing multiple therapists, services or appointments, you’ll have one person who understands the bigger picture and helps keep everyone working towards the same outcomes.

If you’re a parent or carer, we’ll work alongside you while supporting your young person to build confidence, make choices and develop the independence they need for adulthood.

Start with your goals, not a service

You or your family might begin by looking for an occupational therapist, speech pathologist, physiotherapist, employment coach or another specific service. But most goals involve several areas of life working together.

Building friendships may involve communication and confidence. Preparing for employment may involve travel skills, workplace communication and independence. Managing everyday routines may involve emotional regulation, organisation and support strategies.

That’s why we start by understanding:

  • Your strengths, interests and aspirations
  • What’s important to you
  • What you’d like to achieve
  • Your everyday routines and responsibilities
  • Where you learn, work and spend time
  • What’s getting in the way of participation
  • The support that could help you move forward

Together, we’ll build the right team around your goals and the future you want to create.

Who leads your support?

Your youth lead practitioner is your main point of contact and the person responsible for helping coordinate your support.

They are more than a service coordinator. Drawing on their experience supporting young people, they deliver ongoing support, help you build practical skills and bring in therapists, employment practitioners or other professionals when specialist expertise is needed.

Together, you’ll develop one coordinated plan built around your NDIS goals, aspirations and priorities. You remain at the centre of decisions about your support, your team and your future.

With your consent, your youth lead practitioner can also work with your family, school, TAFE, university, employer and other important people in your life.

What can a youth lead practitioner help with?

Depending on your goals, a youth lead practitioner may help you:

  • Build independence and confidence
  • Communicate your needs, choices and ideas
  • Develop friendships and social connections
  • Manage everyday routines and responsibilities
  • Navigate emotions and challenges
  • Participate at school, TAFE, university or work
  • Travel independently and access your community
  • Prepare for employment or further education
  • Navigate important life transitions
  • Make informed decisions about your future.

Support happens in the places where life happens – at home, in education settings, at work and in the community — so you can practice new skills where you’ll actually use them.

Frequently Asked Questions

What is a youth lead practitioner?

A youth lead practitioner is the main practitioner leading therapy and support for a young person aged 9–25. They deliver ongoing support, develop one coordinated plan and bring in allied health, employment and other expertise when needed.

How are parents and carers involved?

Parents and carers can share important information, practise strategies and help create consistency. Their involvement should reflect the young person’s age, preferences, capacity and goals, with the young person involved in decisions wherever possible.

Do I need to know which service I need?

No. Tell us what you want to achieve, and we can help identify the most appropriate starting point. Your team can change as your goals and circumstances change.
