What are NDIS Functional Assessments?

NDIS functional assessments measure your abilities to perform daily life tasks in the everyday places you live, work and play. It looks at your well-being, independence, support needs, mobility, communication, self-management, learning and self-care while performing the tasks.

Everyday Independence offers a range of NDIS functional assessment reports designed to be detailed yet simple, and they cater for diverse circumstances while providing all the necessary details for the NDIS.

Conducted by experienced occupational therapists, the insights from these assessments may help you apply for NDIS funding that better aligns with your goals and quality of life improvement.

Our Suite of NDIS Functional Assessments

Functional Capacity Assessment (FCA)

A comprehensive report that assesses your abilities and needs across 35 essential life tasks including communication, mobility, self-care, social participation, learning and working. This report is for people needing less than six hours of daily support and are not planning on living in a different home in the next few years. Six to eight hours of funding needed to complete.

Home and Living Support Assessment (HLS)

This report assesses your current housing suitability and support needs. For people who need more than six hours of daily support, and whose quality of life would be greatly improved by moving to more suitable accommodation or accessing additional support, such as modifications or domestic assistance. Eight to ten hours of funding needed to complete.

Specialised Disability Accommodation (SDA)

Addressing the unique requirements of Specialised Disability Housing (SDA) funding applications, this report determines your eligibility and preferences for specialised disability housing solutions that are currently not available in the market. The report also includes a timetable of supports you will need. Twelve hours of funding needed to complete.
